In short

  • Yes, it can. Acid or stomach fluid rising past a hiatal hernia can reach the throat and cause a dry cough or hoarse voice.
  • Throat reflux often comes without heartburn. Throat clearing, a lump feeling in the throat and a morning hoarse voice are common signs.
  • Other causes must be ruled out first. An ENT check and reflux tests decide whether the hernia is really to blame before surgery is considered.

Most people link a hiatal hernia with heartburn. But some people come with a cough that will not settle, a voice that turns hoarse, or a need to clear the throat all day. Many of them have no heartburn at all.

This page explains how a hiatal hernia can cause throat symptoms, and how doctors check whether it is really the cause.

How reflux reaches the throat

In a hiatal hernia, the top of the stomach slides up through the opening in the diaphragm. The valve between the food pipe and the stomach then works less well. Acid and stomach fluid can rise up the food pipe.

Sometimes it rises all the way to the throat, often at night when you lie flat. The voice box and throat lining are delicate. Even a small amount irritates them.

  • A hoarse or rough voice, worse in the morning.
  • Clearing the throat again and again.
  • A feeling of a lump or mucus stuck in the throat.
  • A sour or bitter taste on waking.

Living with a cough or hoarse voice and a hiatal hernia on your report? A review can tell you whether the two are linked.

How the link is checked

  1. An ENT doctor looks at the voice box with a thin camera. This rules out a growth and shows any redness from reflux.
  2. An endoscopy shows the hernia and any damage to the food pipe.
  3. A reflux study measures how often fluid rises, and whether it matches your symptoms.

A hoarse voice that lasts more than a few weeks always needs the ENT check, whatever else is found.

When surgery is considered

Most people start with diet changes, raising the head of the bed, and acid tablets for a set time. Many improve.

Surgery is discussed when tests prove that reflux is reaching the throat and symptoms continue despite treatment. The hernia is repaired and a valve is rebuilt by keyhole or robotic surgery. Your surgeon will explain that throat symptoms respond less surely than heartburn, so careful testing comes first.

Frequently asked questions

Can I have reflux in my throat without heartburn?

Yes. Many people with throat symptoms from reflux never feel heartburn. That is why it is sometimes called silent reflux.

Which doctor should I see for a hoarse voice?

Start with an ENT doctor, who looks at the voice box. A hoarse voice that lasts more than a few weeks always needs this check.

Will acid tablets cure my cough?

They help some people. If a full course gives no benefit, the cough may have another cause, and tests are done to find it.

Can hiatal hernia surgery fix a chronic cough?

It can when tests prove reflux is the cause. The results are less sure for throat symptoms than for heartburn, so this is discussed in detail.

What can I do at home for throat reflux?

Eat smaller meals, leave a gap before bed, raise the head end of the bed, cut down tea, coffee and spicy food, and stop smoking.
