In short

  • Diastasis recti is a widening of the gap between the two straight belly muscles. It shows as a long ridge when you sit up.
  • It is not a true hernia. There is no tight opening, so bowel does not get trapped in a muscle gap.
  • Exercise guided by a physiotherapist usually comes first. Surgery is discussed if a wide gap still causes trouble, or a hernia sits in it.

Many people notice a soft ridge running down the middle of the belly when they sit up from lying down. It is common after pregnancy. The first worry is often that it is a hernia.

Usually it is not. It is a muscle gap, called diastasis recti. This page explains the difference and what can be done.

What diastasis recti is

Two long, flat muscles run down the front of the belly, one on each side. They are joined in the middle by a band of tough tissue. When this band stretches, the muscles drift apart and the gap widens.

Pregnancy stretches it most. Twins, more than one pregnancy, a lot of belly fat and heavy straining add to it.

Why it is not a true hernia

A hernia is a hole in the belly wall. Fat or bowel pushes out through it and can get stuck. In a muscle gap there is no hole. The middle layer is thin and stretched, but it is still whole.

That is why a gap on its own does not trap bowel. It can still cause a bulge, back ache, or a feeling that the core is weak.

Noticed a ridge down your belly when you sit up? An examination and an ultrasound can tell a gap from a hernia.

How the doctor tells them apart

  1. You lie on your back and lift your head a little. The doctor feels the gap with the fingers.
  2. The doctor checks the belly button and the scar lines for a separate lump.
  3. An ultrasound measures the gap and shows any hernia sitting inside it.

What helps first

Most people start with guided core exercises. A physiotherapist can teach which moves help and which ones push the belly out. Losing extra weight and treating constipation also help.

After pregnancy, the gap often gets smaller over the first months on its own.

When surgery is discussed

Surgery is considered when a wide gap still causes a bulge or pain after a fair trial of exercise, or when a hernia sits in the gap. The surgeon stitches the two muscles back together and repairs any hernia at the same time.

This can be done by keyhole or robotic surgery, through small cuts. It is a planned operation, never an emergency one.

Frequently asked questions

Is diastasis recti dangerous?

No. A muscle gap has no tight opening, so bowel cannot get trapped in it. It can still cause a bulge, back ache or a weak feeling in the core.

Can I have diastasis recti and a hernia together?

Yes. A small hernia at the belly button often sits in the middle of a muscle gap. An ultrasound shows both.

Can diastasis recti close with exercise?

Often the gap gets smaller, and the core gets stronger, with the right exercises. A physiotherapist can guide you, especially after pregnancy.

Do men get diastasis recti?

Yes. It is seen in men with a lot of belly fat, after big weight changes, and with heavy lifting that strains the belly.

When is surgery done for diastasis recti?

Surgery is discussed when a wide gap still causes trouble after exercise, or when a hernia sits in it. The muscles are brought back together.
