In short

  • A small lump between the belly button and breastbone is often an epigastric hernia. A fatty lump or skin cyst can look similar.
  • A hernia may grow when you cough and may hurt. A fatty lump is soft, moves under the skin and stays the same.
  • A long ridge that shows when you sit up is usually a muscle gap called diastasis. It is not a hernia.

The middle line of the belly, from the breastbone down to the belly button, is a common place to find a lump. Many people notice it while bathing or when a shirt rubs.

Most of these lumps are harmless. A few are small hernias. This page explains how to tell them apart and when it matters.

What an epigastric hernia is

The muscles on each side of the belly join in the middle along a firm band. Small gaps can form in this band. A little fat from inside can push through and form a lump.

This is an epigastric hernia. It is often the size of a pea or a grape. It may be tender, especially after a meal or when you bend. Unlike a groin hernia, it often does not go back in when you lie down, because the fat is held by a narrow gap.

Found a lump above your belly button? A short visit and, if needed, an ultrasound will tell you what it is.

Simple checks you can do at home

  1. Stand up and look at the lump in a mirror. Note its size.
  2. Cough firmly with a hand resting on it. Feel whether it pushes out.
  3. Lie flat and lift your head and shoulders. Watch for a long ridge down the middle.
  4. Pinch the skin. A cyst moves with the skin. A fatty lump slides under it.

These checks point the way. They do not replace an examination.

Why a muscle gap is not a hernia

After pregnancy, or with weight gain around the belly, the two long muscles can drift apart. When you sit up, a soft ridge rises along the middle. This is diastasis.

The band between the muscles has stretched but has not torn. Nothing can get trapped in it. It is treated with exercise and, in some people, surgery for comfort or shape. A true hernia can sit inside a diastasis, which is why an examination helps.

When a scan helps

A small hernia can be hard to feel through a thick belly wall. An ultrasound shows whether there is a gap and what is pushing through it. It also shows whether there are other small gaps nearby, which changes how a repair is planned.

A CT scan is rarely needed for a lump this small.

Does it need repair?

An epigastric hernia does not close on its own. A small one that does not hurt can be watched. Repair is usually advised when it hurts, grows, or when there are several gaps close together.

Fatty lumps and cysts are removed only if they grow, hurt or bother you. A muscle gap on its own is not an emergency.

Signs that need urgent care

Signs that need urgent care

  • The lump becomes hard, very tender and red.
  • You start vomiting or your belly swells.
  • A lump that used to change size now stays fixed and painful.

Frequently asked questions

What is the small lump between my belly button and breastbone?

It is often an epigastric hernia, a small piece of fat pushing through a gap in the middle line of the belly wall. A fatty lump or a skin cyst can look similar.

How can I tell a hernia from a fatty lump?

A hernia may get bigger when you cough or strain and may hurt. A fatty lump feels soft, moves under the skin and does not change with coughing.

Is a ridge down the middle of my belly a hernia?

A long ridge that appears when you sit up is usually a gap between the muscles, called diastasis. It is not a hernia and does not trap bowel.

Do I need a scan for a lump above the belly button?

Often an examination is enough. An ultrasound helps when the lump is small, the belly wall is thick, or the cause is not clear.

Will an epigastric hernia close on its own?

No. The gap does not close by itself. A small hernia that does not hurt can be watched. One that hurts or grows is usually repaired.
