In short

  • A bulge beside a stoma is a common kind of hernia. Many people manage it well with a fitted support belt and a better bag.
  • Repair is considered when the bag keeps leaking, the skin breaks down, it hurts, or the bowel gets stuck in the bulge.
  • A stoma that turns dark, stops working with vomiting, or a hard painful bulge needs hospital care the same day.

A stoma is an opening on the belly where the bowel empties into a bag. It may be a colostomy or an ileostomy. Many people with a stoma later notice a swelling around it.

This is called a parastomal hernia. It is common, and it is often manageable without an operation. This page is about living with it and knowing when to act.

Why the bulge forms

To make a stoma, the surgeon brings the bowel out through a hole in the belly muscles. That hole is a weak point by design. Coughing, lifting and weight gain stretch it over time.

Fat or a loop of bowel then pushes out beside the stoma. The bulge is often bigger when you stand or cough and smaller when you lie down.

Support belts and garments

A stoma support belt or high-waisted garment holds the bulge in and makes the bag sit flatter. It needs an opening for the bag to pass through.

  1. Ask a stoma nurse to measure you. A belt that is too tight can press on the stoma.
  2. Put it on lying down, when the bulge is smallest.
  3. Wear it whenever you are up and active, and always when lifting.

Getting the bag to fit again

A bulge changes the shape of the skin around the stoma. A flat, stiff flange may lift at the edges and leak. A flexible or curved flange, a seal ring, or a different hole size often fixes this.

Measure the stoma again every few months. It can change size as the bulge grows.

When repair is considered

1
The bag leaks again and again in spite of a fitted flange.
2
The skin around the stoma keeps breaking down.
3
The bulge aches or drags through the day.
4
The bowel has got stuck in the bulge, even once.

If the stoma was meant to be temporary, closing it may treat the hernia at the same time. If the stoma is permanent, the gap beside it is closed and reinforced with mesh. Repairs here are more complex than other hernias, and they can come back, so the decision is weighed carefully.

Lifting and daily life

Avoid heavy lifting, especially from the floor. Bend your knees, keep the load close and breathe out as you lift. Treat a long cough or constipation, because both push on the gap. Gentle walking and normal daily work are fine.

Warning signs that cannot wait

Warning signs that cannot wait

  • The stoma turns dark red, purple or black.
  • No output into the bag for many hours, with cramps or vomiting.
  • The bulge becomes hard, tender and will not soften when you lie down.

Go to the nearest hospital emergency department. Do not wait for a clinic visit.

Frequently asked questions

Why has a bulge appeared around my stoma?

The bowel comes out through an opening in the belly muscles. Over time this opening can stretch, and bowel or fat pushes through beside the stoma.

Does a bulge around a stoma always need surgery?

No. Many people manage well with a support belt and a better-fitting bag. Repair is considered when the bag keeps leaking, it hurts, or the bowel gets stuck.

Can my stoma be closed instead of repairing the hernia?

Sometimes. If the stoma was meant to be temporary and you are fit, closing it can treat the hernia too. Your surgeon will check whether that is possible.

What kind of support belt should I use?

A stoma support belt or garment with an opening for the bag. A stoma nurse should measure you, because a poor fit can press on the stoma.

When is a stoma bulge an emergency?

Go to hospital if the stoma turns dark or black, stops working and you vomit, or the bulge becomes hard and very painful.
