Yes, in some cases. A small groin hernia that does not hurt and slips back easily can often be watched safely.
Watching means an agreed plan with review dates, not simply doing nothing and hoping the bulge settles down on its own.
Pain, a bulge that will not go back, or steady growth are the three things that move the plan towards repair.
Being told you have a hernia does not always mean being booked for an operation that week. For some small hernias, watching is a proper plan with a name, and it is one your surgeon may offer.
It is not the same as being sent away. The difference matters, so it is worth being clear about it.
What watching actually means
Watching means you and your surgeon agree that repair is not needed today, and you agree how you will know if that changes.
A real plan has three parts. You know which signs to act on. You have a review date. You have a way to reach the clinic if something shifts before then.
Without those three parts it is not watching. It is drift, and drift is how a small hernia becomes an emergency at an awkward moment.
Which hernias can reasonably be watched
The hernias that suit watching are the quiet ones. A small groin or navel hernia that does not hurt, that slips back when you lie flat, and that has not changed in months.
Studies of men with small groin hernias that caused no symptoms found that waiting was reasonably safe over several years. Many of those men later chose repair anyway, because the hernia grew or began to ache. Waiting delayed the operation; it did not usually replace it.
Which hernias should not be watched
Some hernias are repaired on the diagnosis rather than on the symptoms.
A femoral hernia sits in a tight, unforgiving gap low in the groin, and it has a higher chance of trapping bowel. An obturator hernia is similar. A hernia that will not go back is already partly stuck. A hernia causing pain most days is telling you something.
In those situations the question is when to repair, not whether.
What pushes a hernia towards watching, and what pushes it towards repair
Finding
Watching is reasonable
Repair is advised
Pain
No pain, or a mild ache after a long day
Pain most days, or pain that limits work
Reducing
Goes back on its own when you lie down
Will not go back, or needs force
Size over time
Unchanged over months
Growing visibly month by month
Your work
Desk or light work
Heavy lifting you cannot avoid
Type of hernia
Small groin or navel hernia
Femoral or obturator hernia, at any size
Not sure whether yours can wait? Bring any scan report to the visit and we can set a plan.
Three changes should bring you back sooner than your review date.
The bulge becomes painful rather than just present. The bulge stops going back when you lie down. The bulge is clearly bigger than it was.
Any one of those is a reason to be seen, not a reason to wait for the appointment you already have.
Signs that need care the same day
Watching has one hard limit. If the hernia becomes hard and very painful, will not reduce at all, and you begin vomiting or cannot pass wind, the bowel inside may be trapped.
That is a hospital visit the same day. It is uncommon, but it is the event the whole plan is built around, so it is the part to remember.
Why your own situation matters
Two people with the same sized hernia can get different advice, and both can be right.
Work comes into it. If you lift for a living and cannot avoid it, the hernia gets loaded every day. Age and fitness come into it, in both directions: a long wait may mean repair later in life, when recovery is slower.
Travel comes into it too. If you spend months somewhere without surgical care nearby, a hernia you would otherwise watch may be better repaired before you go.
How to make watching work
Keep it simple. Note the date you were told about the hernia. Note what it looked like then. Check it now and again while lying down, so you know your own baseline.
Treat constipation and a long-standing cough, because both load the wall. Keep your weight steady. If you smoke, stopping helps the wall and helps the repair if you need one later.
Then keep your review date. A watched hernia that is never reviewed is just an unrepaired hernia.
Frequently asked questions
Can a small hernia be left alone?
Sometimes. A small hernia that does not hurt and slips back easily can be watched under an agreed plan with review dates.
How long can I wait before hernia surgery?
There is no fixed limit. Many people watch a quiet hernia for years, but pain, growth or a bulge that will not reduce ends the wait.
Which hernias cannot be watched?
Femoral and obturator hernias are usually repaired on diagnosis, as are hernias that hurt most days or will not go back.
Will waiting make the operation harder later?
It can. A hernia that grows over years is a larger repair than the same hernia treated when it was small.
