In short

  • A lump that appears on standing and flattens on lying down behaves like a hernia. Most other groin lumps do not.
  • Lymph nodes, a vein swelling, a lipoma and a cyst all sit in the groin and are mistaken for hernias.
  • An ultrasound is usually enough to tell them apart. It is done standing and straining, or it can miss a hernia.

Most people who find a groin lump assume it is a hernia. Often it is. Often enough it is not, and the other causes are treated quite differently.

One simple observation separates most of them. Watch what the lump does when you lie flat.

The lying down test

A hernia is contents pushed out through a gap. Lie down and gravity takes them back. The lump flattens, sometimes with a gurgle.

A lymph node, a lipoma or a cyst sits where it is. Lying down changes nothing, because nothing has come through anything.

One exception catches people out. A saphena varix, which is a swelling of a leg vein, also empties on lying down. It has a soft, blue look and other leg veins are usually visible.

Six causes. How the lump behaves on lying down separates most of them.

The other causes, briefly

Lymph nodes swell when something lower down is infected. Look for a cut, an ingrown nail or athlete's foot on that leg. A node that keeps growing, feels hard or stays for weeks needs investigation.

Lipomas are fatty lumps. Soft, sore-free, unchanged by coughing.

Cysts of the cord or canal are smooth and fluid filled. They can look very like a small hernia on examination.

In a young man with an empty scrotum on that side, a groin lump may be a testis that never came down. That is worth naming early.

A lump that does not flatten on lying down still needs an answer. Bring any earlier scan to the visit.

When a scan is needed

If the examination is clear, no scan is needed. If it is not, ultrasound is the first choice.

It has to be done properly. Standing, and while you strain. An ultrasound taken lying still can miss a hernia that only appears on standing, and a normal report then tells you very little.

Frequently asked questions

Can a hernia feel hard?

Yes. A hernia that has become stuck feels firm and is often tender. A hard, tender lump that will not go back is a reason to be seen the same day.

Does a lump that disappears when I lie down mean it is a hernia?

Usually, but not always. A saphena varix, which is a swollen leg vein, also empties on lying down. It looks soft and slightly blue, and other varicose veins are often visible.

Is an ultrasound always needed?

No. A typical hernia can be diagnosed by examination alone. A scan is added when the findings are unclear, when the lump is small, or when another cause is suspected.

Can a lymph node in the groin be serious?

Most swollen nodes follow an infection lower down the leg and settle. A node that keeps growing, feels hard, or stays for several weeks should be investigated properly.

Can I have a hernia and another lump together?

Yes, and it is not rare. Finding a small hernia does not mean it explains the lump you noticed, so both are examined before anything is planned.
