In short

  • In a large or rolling hiatal hernia, part of the stomach moves up into the chest beside the food pipe, not just above it.
  • It often causes fullness after small meals, chest pressure, breathlessness or a low blood count, sometimes with little or no heartburn at all.
  • Severe chest or upper belly pain with retching and no vomit needs a hospital the same day. The stomach may be twisted or trapped.

Most hiatal hernias found on endoscopy are small. They cause heartburn and are often managed with diet and tablets.

A large hiatal hernia, especially the rolling type, behaves differently. This page explains how it differs and why surgeons more often advise repair.

Two kinds of hiatal hernia

The food pipe passes through a gap in the diaphragm to reach the stomach. In the common sliding type, the point where the food pipe joins the stomach slides up through this gap.

In the rolling type, also called paraoesophageal, part of the stomach itself rolls up and sits beside the food pipe in the chest. In some people most of the stomach moves up. This is sometimes called an upside-down stomach.

Symptoms it can cause

1
Feeling full after a few bites.
2
Pressure or pain in the chest after meals.
3
Breathlessness after eating, as the stomach presses on the lung.
4
Food sticking or bringing food back up.
5
Tiredness from a low blood count, due to slow bleeding in the trapped stomach.

Some people have very little heartburn. The hernia may be found on a scan done for another reason.

Warning signs

Rarely, the stomach in the chest twists or gets trapped. Its blood supply can be cut off. Signs include sudden severe chest or upper belly pain, retching without bringing anything up, and being unable to swallow even water.

These need a hospital the same day. Tell the team you have a hiatal hernia.

Why repair is more often advised

Tablets can ease reflux, but they cannot bring the stomach back down. A large hernia usually does not shrink on its own, and symptoms often grow over time.

Your surgeon weighs your symptoms, age and general health. For many people with symptoms, a planned repair is safer than waiting for an emergency. In the operation, the stomach is brought down, the sac is removed, the gap is narrowed and a wrap is often added.

Frequently asked questions

What is a rolling hiatal hernia?

Part of the stomach rolls up through the gap in the diaphragm and sits beside the food pipe in the chest, instead of only sliding up.

Is a large hiatal hernia dangerous?

It can be. The stomach in the chest can twist or get trapped. This is uncommon, but it is an emergency when it happens.

Can I have a large hiatal hernia without heartburn?

Yes. Many people have fullness, chest pressure, breathlessness after meals or low blood count instead of heartburn.

Will acid tablets fix a large hiatal hernia?

They can ease reflux, but they do not bring the stomach back down. Only surgery repairs the hernia itself.

Does every large hiatal hernia need surgery?

Not always. Your surgeon weighs your symptoms, age and general health. Repair is more often advised than for a small hernia.
