In short

  • Most people can have sex again once comfortable, often one to two weeks after a keyhole hernia repair. Pain is the most reliable guide.
  • Bruising and swelling of the scrotum or penis after a groin repair are common. They look alarming but usually settle within two to three weeks.
  • Hernia repair does not affect erections. Damage to the tube that carries sperm is rare, but men planning children should mention it before surgery.

This is one of the questions people want answered and rarely ask. The short answer is reassuring. Sex does not put a hernia repair at risk in the way heavy lifting does, and the operation does not affect sexual function.

What follows covers timing, the bruising that worries many men after a groin repair, and what the operation means for fertility.

How soon is it safe?

There is no fixed number of days. Most people can have sex again once they can move around without much pain. After a keyhole repair that is often one to two weeks. After a large open repair or an incisional hernia repair, it may take longer.

Let comfort be your guide. If something hurts, stop, and try again in a few days. Nothing is harmed by waiting a little longer.

Making the first weeks easier

  1. Choose positions where your partner takes more of the movement.
  2. Avoid positions that press on the groin or the wounds.
  3. Avoid pushing hard with your belly or holding your breath.
  4. Take your usual pain tablet beforehand if it helps.

Bruising and swelling after a groin repair

After an inguinal hernia repair, many men are startled to find the scrotum or penis bruised purple or swollen a few days later. Blood and fluid from the operation simply drain downwards with gravity.

It looks worse than it is. It usually fades over two to three weeks. Firm, supportive underwear and rest help. If the swelling is hard, very painful and keeps growing, ring the clinic the same day.

Hernia repair and fertility

In men, the tube that carries sperm from the testis, called the vas, runs through the groin right next to an inguinal hernia. During the repair it is found and protected.

Injury to it is rare. But it matters most when hernias on both sides are repaired, or when a repair is being redone. If you are planning a family, say so before surgery. It is part of how the operation is planned, not a reason to avoid it.

Erections and sensation

The nerves that control erections are deep in the pelvis. They are not in the area of a groin or belly hernia repair, so the operation does not affect them.

Some men notice a patch of numbness in the groin or at the top of the scrotum. It comes from small skin nerves near the repair and usually improves over months. Early difficulty with erections is most often down to pain, tiredness or worry, and settles as you recover.

For women

The same rule applies. Resume when you are comfortable, and avoid pressure on the wounds for the first few weeks. A hernia repair does not affect a woman's fertility. If you are planning a pregnancy soon, mention it, as it can change the timing of some repairs.

Frequently asked questions

How soon can I have sex after hernia surgery?

Most people can once they are comfortable, often after one to two weeks for a keyhole repair. A larger open repair may need longer. Let pain be the guide.

Can sex damage my hernia repair?

Sex itself does not strain the repair the way heavy lifting does. Avoid positions that press on the groin or need hard pushing in the first few weeks.

Is it normal for the scrotum to swell after inguinal hernia surgery?

Yes, bruising and swelling of the scrotum or penis are common after a groin repair. They look worse than they are and settle over two to three weeks.

Can hernia surgery affect fertility?

Rarely. The tube that carries sperm runs through the groin, so it is protected during the repair. Men planning children should mention it, especially before repairs on both sides.

Does hernia surgery affect erections?

No. The nerves that control erections are not in the area of an inguinal or abdominal hernia repair. Pain and worry are the usual cause of early trouble.
