In short

  • In liver cirrhosis the fluid in the abdomen, not the hernia itself, is what decides whether and when to operate.
  • Ascites raises the pressure inside the abdomen and pushes the navel out, so the hernia grows as the fluid builds up.
  • Skin over the hernia can thin, ulcerate and leak fluid. A leak is an emergency, because infection can reach the abdomen.

An umbilical hernia in liver cirrhosis is a different problem. The same hernia in a healthy adult is not.

Fluid in the belly, called ascites, raises the pressure inside it. That pressure pushes the navel out. The hernia grows as the fluid builds up, and the skin over it thins.

The hernia is often the most visible part of the illness. The liver is what decides what can safely be done.

Why these hernias are risky

1
The skin can thin, break down and leak fluid. A leak is a route for infection into the belly.
2
Rarely the thin skin gives way. That is an emergency.
3
Bowel can get trapped. This is more likely just after the fluid is drained, when the neck of the hernia tightens.

Each of those is a reason to watch the hernia, even when an operation is not yet on the table.

Why the liver decides

Surgery in cirrhosis carries risks that follow how well the liver is working. Bleeding from poor clotting. Infection. A liver that gets worse. Fluid that leaks through the wound.

The liver reserve is judged on standard scores. Those scores sit at the centre of the decision.

Controlling the fluid comes first. Diet, water tablets, drainage, or a procedure that lowers the pressure in the liver blood flow.

The liver is treated first. The hernia is repaired once the ascites is controlled.

What has to be settled before a planned repair

ItemWhy it comes first
Control of the ascitesA repair done against a full abdomen is under constant load, and the wound leaks or the hernia returns.
Liver reserve, by the standard scoresThey estimate how the liver will cope with an anaesthetic and an operation.
Clotting and plateletsCirrhosis affects both, and bleeding risk is judged on the numbers rather than assumed.
Nutrition and muscle bulkWound healing in cirrhosis is slower, and poor nutrition makes it slower still.
The transplant planIf you are being assessed for transplantation, the timing is agreed with that team.
The state of the skinThin or ulcerated skin over the hernia changes the urgency of the whole decision.

Bring your latest liver function tests, your scan and your hepatologist's notes to the consultation.

Planned repair or waiting

Where the liver is coping and the fluid is controlled, a planned repair is usually safer than waiting. Emergency surgery on this liver is far riskier than a planned one.

Where the liver disease is advanced, the balance can tip the other way. The aim then is to protect the skin and treat problems as they come.

There is no single answer. That is why the surgeon and the liver doctor decide these cases together.

Warning signs that need help the same day

Warning signs that need help the same day

  • The skin over the hernia darkens, ulcerates or leaks fluid.
  • The hernia becomes tense and painful and will not go back.
  • Fever, confusion, or a sudden increase in abdominal pain.

If the hernia is leaking, cover it with a clean dressing and seek medical care the same day.

Frequently asked questions

Why did my hernia appear after my liver disease?

Ascites raises the pressure inside the abdomen and stretches the navel, which is a natural weak point. The hernia usually grows with the fluid.

Can my hernia be repaired while I still have ascites?

The ascites is ideally controlled first. Repair against uncontrolled fluid has a high risk of wound leak and recurrence.

Is it safer to leave it alone?

Not necessarily. Emergency surgery for a complicated hernia in cirrhosis carries higher risk than a planned repair. The decision depends on liver function.

My hernia is leaking fluid. What should I do?

Treat it as urgent. Cover it with a clean dressing and seek medical care the same day.

Will my liver doctor be involved?

Yes. These decisions are made together with the hepatologist, and with the transplant team where relevant.
