In short

  • Heavy lifting rarely causes a hernia on its own. It raises the pressure in the belly and shows up a weak spot already there.
  • Many people are born with a weak spot in the groin. Age, a long cough, constipation, smoking and past operations weaken the wall further.
  • Some causes can be changed. Treating a cough or constipation, stopping smoking and losing extra weight lower the strain before and after repair.

Many people first notice a hernia just after lifting something heavy. It is natural to think the lift caused it. Some even blame a gym session or a day of moving house.

The truth is a little different. This page explains what really causes a hernia, and which causes you can change.

Two things are needed

A hernia forms when two things meet. The first is a weak spot in the belly wall. The second is pressure from inside the belly that pushes against it.

Lifting adds pressure. So does coughing, straining on the toilet, or carrying extra weight. But pressure alone rarely makes a hole in a healthy wall. It usually finds a weak spot that was already there.

Weak spots you may be born with

Before birth, a small passage in the groin lets the testis move down in boys. It normally closes. In some people it stays partly open. Years later, fat or bowel can push into it.

This is why groin hernias are common in young, fit men, and why a hernia can show on the other side later.

What weakens the wall and what adds strain

What weakens the wall and what adds strain

CauseWhat it doesCan it be changed?
Weak spot from birthA small opening in the groin that never fully closedNo, but it can be repaired
AgeMuscles and tough tissue get thinnerNo
Past operationA scar is weaker than the wall around itNo, but good healing helps
Long cough or constipationStrains the belly again and againYes, with treatment
Smoking and extra weightWeakens tissue and raises belly pressureYes

Noticed a lump after lifting something heavy? An examination tells you what it is and whether it needs a repair.

Weak spots that develop later

1
Age thins the muscles and the tough tissue of the belly wall.
2
A scar from an earlier operation is never as strong as the wall around it.
3
Pregnancy stretches the wall around the belly button.
4
Smoking weakens the tissue that holds the wall together.

What you can change

  1. Get a long-standing cough treated.
  2. Treat constipation, so you do not strain on the toilet.
  3. Get help early for straining to pass urine.
  4. Stop smoking, and lose extra weight slowly.
  5. Lift with bent knees, close to your body, and breathe out as you lift.

These steps cannot close a hernia that is already there. They do lower the strain on it, and they help a repair hold once it is done.

Frequently asked questions

Can one heavy lift cause a hernia?

Rarely on its own. A sudden lift often makes a hernia show for the first time, but the weak spot was usually there before.

Is a hernia my fault?

Usually not. Many people are born with a weak spot in the groin. Age, a long cough or a past operation add to it.

Can I keep lifting at work with a hernia?

Light work is usually fine. Heavy lifting can make the lump bigger and more painful, so discuss your job with your surgeon.

Can a hernia be prevented?

Not always. A healthy weight, stopping smoking, treating a long cough or constipation, and lifting with bent knees all lower the strain.

Why did I get a hernia on the other side too?

The weak spot in the groin is often present on both sides. A hernia on the second side is common, sometimes years later.
