In short

  • Hernia pain is most often a dull ache or a dragging, heavy feeling at the lump. It grows through the day and eases lying down.
  • Some people feel burning or a sharp twinge when they cough, lift or bend. Many hernias cause no pain at all, only a lump.
  • Severe pain with a hard lump that will not go back, vomiting or red skin is an emergency. Go to hospital the same day.

People often ask whether their pain sounds like a hernia. Hernia pain has a few common patterns. Knowing them helps you judge when to book a visit and when to go to hospital.

This page describes those patterns. It does not replace an examination.

The usual feel: a dull ache or a drag

Most people describe a dull ache at the lump. Others feel a pull, a weight, or a sense that something is dragging down in the groin.

The feeling tends to build as the day goes on. Standing and walking push more fat or bowel into the hernia. When you lie down, it slides back and the ache fades.

Burning, tingling and sharp twinges

Nerves run close to the groin. A hernia can press on one of them. This may feel like burning or tingling in the groin, and sometimes down the inner thigh or into the scrotum.

A quick, sharp twinge when you cough, sneeze or lift is also common. It comes from the sudden rise in pressure inside the belly.

Living with an ache or dragging feeling in the groin or belly? An examination can tell you whether a hernia is the cause.

Pain with no lump you can see

A small hernia can ache before a lump shows. Some sit deep in the wall, like a femoral or Spigelian hernia. A doctor may feel it when you cough. If not, an ultrasound scan can help.

Groin pain also has other causes, such as a muscle strain or a hip problem. An examination sorts these out.

Warning signs: when the pain means hospital

Warning signs: when the pain means hospital

  • The pain is severe and does not ease when you lie down.
  • The lump turns hard or tender and will not go back in.
  • The skin over it turns red or dark.
  • You vomit, or cannot pass wind or stool.

These can mean the bowel is trapped. Go to a hospital the same day.

Does pain mean the hernia needs repair?

A hernia that hurts or limits your day is a common reason to plan a repair. A hernia that does not hurt may still be repaired, because most grow slowly over time.

Your surgeon looks at the type of hernia, your health and your work, and then talks you through the choice.

Frequently asked questions

Does a hernia always hurt?

No. Many hernias cause no pain at all, only a soft lump. Others ache now and then, often after a long day on your feet.

Can I have hernia pain without a lump?

Yes. A small hernia may cause an ache before any lump can be seen. A doctor can often feel it, or a scan can show it.

Why does my hernia hurt more in the evening?

Standing and moving all day push more tissue into the hernia. The pressure builds, so the ache is often worse by evening.

Is sharp hernia pain dangerous?

A brief twinge when you cough or lift is common. Pain that is severe, lasting, and with a hard lump needs a hospital the same day.

Does the size of the hernia decide the pain?

Not always. Small hernias with a tight opening can hurt more than large ones. Pain alone does not tell how big a hernia is.
