In short

  • For most men, no. The tube that carries sperm lies next to the hernia. Surgeons protect it, and lasting harm to fertility is uncommon.
  • The risk needs more thought when both sides are repaired, when it is a repeat operation, or when a man has only one working testicle.
  • Hernia repair does not change testosterone or erections. The testicle keeps making hormones because its blood supply is protected during surgery.

Many men who need a groin hernia repair are young and plan to have children. A common worry is whether the operation could affect this.

For most men, the answer is no. This page explains why, and when it needs more thought.

Why the question comes up

Sperm is made in the testicle. It travels up a thin tube, called the vas deferens, through the groin. The tube and the blood vessels to the testicle run right next to an inguinal hernia.

During repair, the surgeon separates the hernia from the tube and the vessels. They are handled gently and left in place.

What the research shows

Most studies find that a hernia repair on one side does not lower a man's fertility. Damage to the tube is uncommon.

A mesh lies near the tube. In a small number of men, scar tissue around the mesh can narrow it. Research on this is still going on.

Open and keyhole repair both protect the tube. Neither has been clearly shown to be safer for fertility.

When it needs more thought

Tell your surgeon if you plan to have children, and if any of these apply.

  • Both sides need repair.
  • The hernia has come back after an earlier operation.
  • You have only one testicle, or one that does not work well.
  • You already have a known fertility problem.

In these cases, a semen test before surgery can help. It gives a clear starting point.

Hormones and erections

The testicle makes testosterone as well as sperm. Its blood supply is protected during the operation, so hormone levels do not change.

Erections are not affected by a hernia repair. Some swelling of the scrotum is common in the first weeks and settles.

If you are trying for a baby

A hernia repair does not need to delay plans for a family. Sex is usually comfortable again within two to three weeks.

If a couple has been trying for a year without success, both partners should see a fertility doctor. Mention any past hernia repair, including one done in childhood.

Frequently asked questions

Can hernia mesh cause infertility?

It is uncommon. In a small number of men, scar tissue around the mesh can narrow the sperm tube.

Should I have a semen test before hernia surgery?

It can help if both sides need repair, it is a repeat repair, or you have only one working testicle.

Does hernia surgery lower testosterone?

No. The blood supply to the testicle is protected, so it keeps making hormones as before.

Is keyhole or open repair safer for fertility?

Both protect the sperm tube. Research has not clearly shown that one is safer than the other.
