In short

  • A groin hernia that reaches the scrotum has usually grown for years. It is not an emergency by itself, but it should be repaired.
  • Large scrotal hernias get stuck more often and grow harder to repair over time. Repairing them while they still go back is easier.
  • Not every scrotal swelling is a hernia. A hydrocele or a lump in the testis looks similar, and an examination or ultrasound tells them apart.

Most groin hernias start as a small bulge just above the crease of the thigh. Left for long enough, some keep growing downwards until they reach the scrotum. Doctors call this an inguinoscrotal hernia.

Many men live with one for years before they seek help. This page explains why size changes the picture, and what the repair involves.

Why a hernia ends up in the scrotum

In men, the groin has a natural channel through which the cord to the testis runs. A common kind of groin hernia follows that same channel. Each time the pressure in the belly rises, a little more is pushed along it.

Over months and years the channel stretches and the hernia can travel all the way down. It often contains fat, and sometimes a loop of bowel.

Why size changes the urgency

A small hernia that goes back easily is low risk. A large one is different. The more it holds, the more likely a loop of bowel is to get stuck in it.

Large hernias also become harder to repair. The tissues stretch, and the hernia sac sticks to the cord. Repairing it while it still goes back when you lie down is simpler and safer than waiting.

How the repair is chosen

Many scrotal hernias can be repaired by keyhole or robotic surgery. The choice depends on how large the hernia is, whether it goes back, any past surgery in the area, and your general fitness.

Very large hernias, or ones that no longer go back, are sometimes better repaired through an open cut in the groin. The aim is the same in both. The hernia is freed from the cord, returned to the belly, and the weak area is covered with mesh.

This choice is made after examination, not from a photo or a scan report alone.

After the operation

Swelling and bruising of the scrotum are common after repair of a large hernia. Fluid can collect in the space the hernia used to fill. It looks worrying but usually settles on its own over a few weeks.

Firm supportive underwear helps. Skin that was stretched by a large hernia tightens slowly over the following months.

Warning signs that need same-day care

Warning signs that need same-day care

  • The hernia has become hard and tender and will not go back.
  • The skin over it has turned red or dark.
  • You are vomiting, or your belly is swollen and you cannot pass wind.
  • Sudden severe pain in the testis.

Frequently asked questions

Is a hernia in the scrotum dangerous?

It is not an emergency on its own, but it should be repaired. Large hernias are more likely to get stuck, and they become harder to repair the longer they grow.

How do I know whether it is a hernia or a hydrocele?

A hernia usually gets bigger on coughing or standing and may go back when you lie down. A hydrocele does not. An examination, and sometimes an ultrasound, settles it.

Can a large scrotal hernia be repaired by keyhole or robotic surgery?

Often, yes, but not always. Very large hernias, or ones that do not go back, are sometimes better repaired open. The choice is made for each patient.

Will the scrotum go back to normal after repair?

Swelling after the operation is common and takes weeks to settle. Skin that was stretched by a large hernia slowly tightens over the following months.

What if I find a lump in the testis itself?

A lump in the testis is different from a hernia and should be checked promptly, usually with an ultrasound. Do not assume it is part of the hernia.
